Community Health Needs Assessment(s)

The Clay County Health Department (CCHD) completed a comprehensive community health assessment in Clay County in 2023. The project involved surveying both residents and employees of the county, conducting extensive secondary data research, analyzing the data, and producing a report outlining key findings and recommendations.


Goal: Effectively plan and conduct a comprehensive Community Health Assessment for the Clay County Health Department.


Objective: Community Health Needs Assessment (CHNA) goes beyond data collection. Its main goal is to motivate change. This change aims to provide a healthy, safe community environment that facilitates healthier community members. These goals are accomplished by providing citizens, community stakeholders, and the health department with an appraisal of community members' well-being through economic status, education level, general health, and welfare. The CHNA provides a means to expand community partnerships, community engagement, and developing goals. These goals are centered on providing a healthier community for all citizens of all health statuses.

The Clay County Health Department (CCHD) enlisted the assistance of Kingery and Company, LLC, to conduct a community health assessment in Clay County. The project included surveying residents, conducting secondary data research, facilitating an analysis retreat, and providing a summary report with key findings and recommendations.


Goal: Plan and conduct an all-inclusive Community Health Assessment for Clay County Health Department.


Objective: 1) Collect primary data by gathering external community feedback through quantitative surveys and qualitative questionnaires with key stakeholders, etc. 2) Collect secondary data by researching national, state and local health data resources. 3) Facilitate an analysis process that identifies priority health issues and mobilizes CCHD and community resources in a plan to address the issues.
